What exactly does NEMT insurance cover for Florida fleets?

Non-Emergency Medical Transportation (NEMT) insurance covers specially equipped vehicles carrying patients to medical appointments. Beyond standard commercial auto coverage, it includes critical general liability and professional liability to cover passenger injuries that occur during boarding, transit, or off-boarding.

NEMT Company vs. Transportation Network Company

Both carry passengers for pay, but Florida treats them differently, and so do the policies.

A NEMT company

Arranges or provides non-emergency medical rides for Medicaid riders under a contract with the state or a health plan. Florida does not treat that arrangement as a transportation network company (§ 627.748(1)(e)). Your contract and your commercial auto policy set the coverage.

A transportation network company

Runs an app that matches riders with drivers, and the statute sets coverage for each phase of the trip. If that is your business, read our TNC insurance page instead. A van that serves both kinds of work needs a policy written for that mix.

Florida NEMT Insurance Requirements

No single statute sets one NEMT limit. The limit you carry comes from these places.

Where the limit comes fromWhat it means for your company
The broker, health plan or agency that hires youYour contract names the liability limit, who must be listed on the policy and how fast you must send proof. Read it before you buy.
Florida's transportation disadvantaged program (Fla. Admin. Code 41-2.006)A Community Transportation Coordinator must ensure at least $200,000 per person and $300,000 per incident on the trips it buys or provides. Operators who drive for one see that figure in their contract.
Federal motor carrier rules (49 C.F.R. § 387.27)They apply to for-hire motor carriers that carry passengers across state lines. Most Florida NEMT trips stay inside the state.
Florida's regular auto lawsEvery vehicle in the fleet carries the state's required coverage. Our commercial auto insurance page lists those minimums.

These are the sources of your limit, not legal advice or a quote. Your contract can ask for more.

What NEMT Business Insurance Covers

A NEMT policy has to protect the vans, the riders and the crews. Most Florida operators build it from these four parts. Many rides end at an assisted living facility or an adult day care center, and those centers have pages of their own: assisted living facility insurance and adult day care insurance.

Auto liability and physical damage

Injuries to riders and other people, and damage to property, when a van crashes. Physical damage repairs the van and its lift. Every contract asks for this one first.

Hired and non-owned auto

Covers claims when drivers use their own cars or you rent a van for a busy week. This is the gap that surprises small NEMT companies.

General and professional liability

Claims from the service itself, such as a rider hurt getting in or out of a van, or a missed pickup. Some contracts ask for both.

Workers compensation

Drivers who are employees are covered for work injuries. Helping riders in and out is physical work.

How do you choose among NEMT insurance companies?

Choose the NEMT insurance company whose policy matches your contract, not only on price. Brokers and health plans name their own limits, additional insureds and proof deadlines, and carriers accept those terms in different ways. From its Kendall office, American US Insurance reads that contract with you first. Once the contract is read, we check which non-emergency medical transportation insurance carriers will quote your vans and drivers on every line, for operators in Miami, Hialeah, Fort Lauderdale, Orlando and Tampa.

Common Questions

NEMT Insurance Questions

What commercial insurance does a Florida NEMT business need?

NEMT operators require specialized commercial auto liability and physical damage for their outfitted vans. A complete package also includes general liability, professional liability (for passenger loading/unloading incidents), and workers compensation for drivers. We build the policy limits directly around the requirements dictated by your broker contracts.

How much does Florida NEMT fleet insurance cost?

NEMT is a high-liability class, so pricing relies heavily on the size of your fleet, MVRs, total mileage, and the required limits. A startup fleet with zero loss history and drivers with minor violations will face steep premiums. We combat this by comparing your risk profile across multiple A-rated NEMT carriers simultaneously.

Why is NEMT commercial insurance so expensive in Florida?

Underwriters view vehicles transporting vulnerable patients as an extreme liability risk, where a single crash can result in multiple severe medical claims. To lower your rate, you must implement rigorous driver screening, mandate safety training, and maintain meticulous maintenance logs—data we highlight when marketing your account to carriers.

What are the legal insurance requirements for NEMT in FL?

There is no single universal limit. Your required limits are dictated entirely by your health plan contracts, Florida's transportation disadvantaged program rules (if you drive for a state coordinator), or federal FMCSA rules if you cross state lines. We read your specific contracts to ensure your policy meets every mandatory threshold.

How do I start a NEMT business with the correct insurance?

Start by acquiring the enrollment rules from your target NEMT broker, as they will explicitly state the required liability limits. Next, provide us with your vehicle VINs (noting wheelchair lifts) and driver licenses. We will bind a fully compliant policy and issue the necessary proof of coverage so you can accept your first dispatch.

What is a NEMT broker, and does it affect my insurance?

A NEMT broker is a dispatch management company that coordinates rides for hospitals and Medicaid, hiring independent providers like you to execute the trips. They will fiercely audit your insurance limits before authorizing dispatch. (Do not confuse them with an insurance broker—we are the agency that secures the policy to satisfy their audit).

Why compare NEMT insurance providers before you sign?

NEMT startups face extreme underwriting volatility. One carrier may outright decline a new venture, while another specializes in it. Additionally, coverage for critical mechanisms—like hydraulic wheelchair lifts—varies drastically between policy forms. Our independent agency compares these critical exclusions so you are fully protected during boarding and transit.

Can a brand new NEMT startup get a quote from your agency?

Yes. We actively insure NEMT startups operating just two or three vans across Miami-Dade, Broward, and the rest of Florida. We consolidate your commercial auto, general liability, and workers compensation into one cohesive quote, rapidly issuing the required compliance certificates to your healthcare brokers.
